Analysis SummaryThis course challenges student's conceptions of the most effective way to support patients in the self management of chronic conditions such as diabetes, depression and cardiovascular disease. For example, many students initially held on to out-dated concepts based on a medical model in which health professionals essentially direct patients in how to manage their diseases. Research and emerging practice standards emphasize the importance of patient self-management, and the use of client-centered counseling skills, to promote improved health outcomes.

It takes students some time to understand and demonstrate client-centered approaches and skills for chronic disease management.

While just 66% of students passed the mid-term exam, 83% of students passed the final exam.

Given the challenging nature of the concepts and competencies taught in this class, the faculty was pleased with the final exam pass rate. At the same time, faculty were inspired to re-think course content, pedagogy and assessment methods with the goal of supporting students to more quickly master key concepts.
Next Steps PlannedFaculty also consulted with local experts in the field of chronic disease management, including the staff at the San Francisco Transitions Clinic.

Based on all data gathered, faculty decided to revise the Course Outline, including SLOs and content, and to revise mid-term and final exam assessments.

This assessment period also coincided with the the beginning of new federal grant to adapt this course - Health Education 66 - and all courses in the Post Prison Health Worker Certificate Program for online delivery.

Faculty worked to develop an online version of HLTH 66 for delivery in the Summer of 2013. This course development was guided by our SLO assessment and provided an opportunity to update curricula content on issues such as patient self management, medications management, and guidelines for health nutrition and exercise. In addition, faculty developed and piloted new performance-based exams and grading rubrics.
Learning OutcomesFaculty will work with the Coordinators of all three CTE Programs that require HLTH 66: the Post Prison Health Worker, Infectious Disease, and Medical Evaluation Assessment Programs to revise the Course Outline. Once a new course outline has been developed that meets with approval from all three programs, it will be submitted to the Curriculum Committee.

We anticipate revising the course outline in the Fall of 2013, and submitting it for approval in the Spring of 2014.

Faculty will also review materials from the online course for adaptation and piloting in the Spring of 2014. This will include new materials - including instructor developed video resources - on topics of self-management, Action Planning, exercise, nutrition and medications management.

Changes

DetailsChanges were made in the Spring of 2013 to the development of a new online section of the course, including the development of new curricula resources.

Faculty developed new content, including video demonstrations of chronic conditions management skills, assignments and exams. While these were for the online environment, most of these materials will be relevant for the traditional or in-person course scheduled to be taught next in the Spring of 2014.
