Assessment Descriptionall records for individual vaccines administered were reviewed and notice was taken of scheduled vaccines and vaccines that were missed. individuals who missed their vaccines were either called or/and emailed to conduct a survey as to why she/he missed the schedule vaccine:

1. I did not complete the series /recommended doses of vaccines because:

A. I didnot know more doses were needed

B. One vaccination would protect me from the disease

C. I was concerned about the side effects

D. I was never told to come back for additional vaccine

E. I am no longer eligible for the VFC program because:

A. I am no longer a student

B. I turned 19years old

C. I have health insurance that covers vaccines

reasons for missed scheduled vaccines are: forgot, not eligible due to age, and transferred out of CCSF
Outcomes1. for reason student stated "forgot"- implemented program to provide reminder phone calls to students for scheduled vaccines.

2. for reasons student not eligible: gave options for self pay to complete vaccines, ref to outside resources for vaccine completion

3. fro reason student transferred out of CCSF: encouraged student to complete vaccines at new school, outside resources given.

Data Summary18 students were immunized with one or more of the following vaccines: HPV, Menactra, MMR, Adacel, Hepatitis B, hepatitis A.

1. 3 doses of HPV vaccine: 15 students were started on the vaccination. 3 student completed 3 dose vaccine series, 5 students unable to complete 3 dose due to age ineligibilty, 2 forgot scheduled vaccination- but returned but reminder phone call, 1 student did not return PC or email for scheduled dose, 1 student missed dose due to going to another school, 4 students with future scheduled dates of completion

2. MMR- one student received vaccine

3. menactra- 9 students vaccinated

4. 3 doses of heptitis B- 5 students started vaccination: 1 completed , 1 ineligible to complete due to age, 3 has scheduled vaccinations in the future

5. adacel- 5 students vaccinated

6. hepatitis A

multi- dose vaccines poses a problem for students completing the recommended vaccinations.
Data Analysiswe would like 100% completion of the recommended vaccines if the students remain eligible while they are at CCSF. For students who were still eligible, the difficulty was having the students remember for scheduled multi dose vaccines: gardasil, hepatitis A and hepatitis B.
Next Steps Planned

Immediate implementation of reminder calls/emails for scheduled vaccines and provide outside resources for those ineligible.

Learning Outcomes

all eligible students will complete recommended doses of various vaccines while at ccsf and all ineligible students will be given resources to complete recommended vaccine series.
